The 2nd form of ID must have your CURRENT address on it. It can be a CURRENT bill.

You MUST have your 1583 form Notarized and you can then fax it to us at 888-226-0081 with both of your IDs

If you are local you can bring your forms in and have them Notarized for FREE at our location.
